Pet Policy Behavior Criteria

  • Obedient
  • Crate-Trained
  • Near Perfect Recall (comes to the owner on command)
  • Minimal Separation Anxiety (not disruptive when separated from the owner)
  • No Signs Of Aggressive Behavior, Chasing, Excessive Barking, Or Biting
  • Capable Of Staying Leashed Throughout The Day
  • Fully Bathroom Trained
  • Well Socialized And Comfortable Around Other Animals And People
  • Rarely Barks/Makes Minimal Noise 
  • Not a Distraction Or Disruption To Owner Or Coworkers Productivity 

BY COMPLETING THIS AUTHORIZATION, I (THE SUPERVISOR) ACKNOWLEDGES AND REPRESENTS the approval or denial of permitting an employee to bring one’s pet to the workplace; I have conducted an unbiased observation of the pet applicant, and have successfully determined whether the pet would be a positive and non-disruptive addition to the office environment. I understand the Pet Policy in full and agree to follow the pet strike policy, communicating warnings and administering strikes on an unbiased and consistent disciplinary basis. If the pet is a disruption or nuisance to the office, I will request the employee to remove their pet from the office immediately and understand that I may revoke a pet agreement at any time if the situation isn't working out. I understand that the observations and comments in this form may be shared with the employee and HR confidentially if the request to bring the pet to work was denied.
